A chipped tooth from a fall at the playground, a knocked-out tooth from a bike accident, or a cracked molar from a hard hit during a game, dental trauma in children can happen in the blink of an eye. The good news is that prompt, appropriate care makes a significant difference in protecting your child’s smile and their developing teeth. Knowing where to turn and what to expect helps you stay calm and act fast when it counts most.

At Wellness Pediatric Dentistry & Airway, we approach dental trauma with the same philosophy that guides everything we do: transparent, minimally invasive care that respects your child’s developing body. As San Antonio’s first holistic pediatric dental practice and a Diplomat of the American Board of Pediatric Dentistry, Dr. Christi Wengler brings advanced training and a genuine commitment to your child’s long-term health. Our emergency care services are developed to address dental injuries quickly, thoroughly, and gently, so your child feels safe from the moment they walk through our doors.

Common Types of Dental Trauma in Children

Dental trauma covers a wide range of injuries, and children experience them more often than most parents expect. The American Academy of Pediatric Dentistry notes that traumatic dental injuries are among the most common dental emergencies in young patients, and its guidelines for managing injuries in the primary dentition were updated in 2020 to reflect current best practices for immediate and urgent care.

The most common types of dental trauma we treat include fractures (chipped or broken teeth), luxations (teeth that have been pushed out of position), and avulsions (teeth that have been completely knocked out). Soft tissue injuries to the lips, tongue, or gums often accompany these, and each situation requires a careful, case-by-case evaluation. Even injuries to baby teeth deserve immediate attention, as damage to primary teeth may affect the underlying permanent teeth still developing beneath the gums.

What to Do Right Away

If your child gets a dental injury, staying calm is the first step toward a better outcome. Here is what to do while you make your way to our office:

  • For a knocked-out permanent tooth, handle it only by the crown (not the root), rinse it without scrubbing, and keep it moist in milk or your child’s saliva until you arrive. If you feel comfortable, you can gently re-implant the tooth in the socket- this gives the tooth the best chance of survival.
  • For a chipped or fractured tooth, rinse your child’s mouth with warm water and apply a cold compress to relieve swelling
  • For a tooth pushed out of position, you can attempt to gently reposition it yourself, as it is easier to reposition in the first minutes after the accident. Then,  contact us right away
  • For soft tissue injuries with significant bleeding, apply gentle pressure with a clean cloth

Time is a critical factor, especially with avulsed permanent teeth. The sooner you reach us, the more treatment options may be available for your child.

How We Treat Dental Trauma at Wellness Pediatric Dentistry

Our approach to dental trauma prioritizes your child’s comfort, safety, and long-term oral health. We begin with a thorough evaluation, including a visual exam and digital imaging when needed, to assess the full extent of the injury. From there, we walk you through every treatment option in clear, simple language, because we believe parents deserve to make informed choices about their child’s care.

Depending on the injury, treatment may involve bonding or composite restoration for fractured teeth, stabilization or splinting for displaced teeth, or, in the case of a knocked-out permanent tooth, re-implantation when timing allows. For injuries involving deeper structures, our restorative dentistry services may include tooth-colored fillings using biocompatible materials that align with our fluoride-free, holistic philosophy. When a child is too young, too anxious, or the injury too complex for treatment in the chair, our sedation dentistry options range from nitrous oxide to general anesthesia, all discussed openly with you before you make any choices.

Protecting Your Child's Smile After a Dental Injury

Recovery from dental trauma does not end at the initial appointment. Follow-up care is essential for monitoring healing, catching complications early, and protecting both the injured tooth and the surrounding structures. We schedule follow-up visits as needed and provide you with clear at-home guidance so you always know what to watch for.

Prevention is also part of the conversation. During routine pediatric dental visits, we discuss age-appropriate strategies for reducing injury risk, from proper mouthguard use during sports to safe play guidance for toddlers. Because most jaw growth happens before age six, early intervention and ongoing monitoring through your child’s preventive dentistry visits give us the best opportunity to protect their development and set a strong foundation for lifelong oral health.
