When a child’s tooth becomes deeply decayed or injured, many parents fear the worst-case scenario of needing to have that tooth removed. Fortunately, that is not always the case. Pulp therapy is a treatment designed to address damage to the innermost part of a tooth, known as the pulp, while preserving the natural tooth structure. Saving a child’s tooth, even a baby tooth, matters far more than most parents realize. Primary teeth hold space for incoming permanent teeth, support speech development, and allow children to chew comfortably. Losing them too early can set off a chain of developmental concerns that affect your child for years to come.

At Wellness Pediatric Dentistry & Airway, we approach pulp therapy with the same minimally invasive, transparency-first philosophy that guides everything we do. Our practice was built on the belief that parents deserve full knowledge of what goes into their child’s mouth and why, and that belief extends to restorative care. We take the time to explain every step of the process, discuss the materials we use, and ensure you feel confident and informed before any treatment begins.

What Is Pulp Therapy for Children?

Pulp therapy is an umbrella term for treatments targeting the dental pulp, the soft tissue inside a tooth that contains nerves and blood vessels. In pediatric dentistry, two primary types are used depending on the severity of the damage.

Pulpotomy

A pulpotomy involves removing only the portion of the pulp within the crown of the tooth, the part you can see above the gumline, while leaving the root pulp intact. This is typically performed when decay has reached the pulp, but the root tissue remains healthy.

Pulpectomy

A pulpectomy, on the other hand, removes the pulp from both the crown and the root canals and is used when infection or damage has spread more extensively throughout the tooth. Both procedures aim to relieve pain, eliminate infection, and keep the tooth functional until it naturally falls out, making room for the permanent tooth beneath it.

According to the American Academy of Pediatric Dentistry, pulp therapy in primary teeth is a well-established clinical approach supported by decades of evidence, and preserving these teeth through appropriate intervention remains a key goal in pediatric dental care.

Signs Your Child May Need Pulp Therapy

Not every child can clearly express that something is wrong with their tooth. As a parent, it helps to know what to watch for. Some of the most common indicators that pulp therapy may be needed include tooth pain that lingers after eating or drinking something hot or cold, spontaneous toothaches without an obvious cause, and visible darkening of a tooth. You may also notice swelling or tenderness in the gums around a specific tooth, or your child may begin chewing on only one side of their mouth.

It is worth noting that some children, particularly younger ones, may not vocalize discomfort at all. Regular dental cleanings and exams allow us to catch early signs of pulp problems before pain becomes severe. Catching these issues sooner rather than later typically means less invasive treatment and a faster, more comfortable recovery for your child.

How We Approach Pulp Therapy at Wellness Pediatric Dentistry & Airway

Our approach to restorative dentistry is rooted in doing only what is necessary and being fully transparent about every aspect of your child’s care. Before beginning pulp therapy, we thoroughly evaluate your child’s tooth using X-rays and a clinical examination to confirm the extent of pulp involvement and determine the appropriate treatment. We discuss our findings with you directly and walk through the recommended approach, the materials involved, and what to expect during and after the procedure.

We understand that many families in Castle Hills and the surrounding San Antonio area prioritize holistic, biocompatible care. That commitment shows up in how we select materials for pulp therapy as well. We favor options that align with our holistic approach to pediatric dentistry that minimize any unnecessary chemical burden on young, developing bodies. If you have questions about specific materials before treatment, we welcome the opportunity to answer them comprehensively.

Keeping Your Child Comfortable

Child comfort is never an afterthought at our practice. We work in calm, private treatment spaces well suited for children who may feel nervous, including those with sensory processing needs. For children who need additional support to stay relaxed during a procedure, we also offer sedation dentistry to make the experience as gentle as possible. Our goal is always to help your child leave our office feeling calm and not afraid, because those early dental experiences shape how they feel about dental care for the rest of their lives.
